Cancellation Request

Are you sure!? We hate losing any of our members, but respect all requests. Let us know if we had a 

shortcoming, or something out of our hands? Please note that if you are under a contract, there is an 

early termination fee of $99, per contract. Per all memberships, there is a 30 day notice required, so you 

will be responsible for 1 more billing cycle after your requests. 

TO SUBMIT YOUR REQUEST YOU MUST DO SO IN WRITING. CERTIFIED MAIL OR EMAIL TO THE APPROPRIATE MANAGER (WHERE YOU SIGNED UP). INCLUDE THE FOLLOWING INFO:

1. Exact date you wish for cancellation to take effect (again - 30 day notice required)

2. Reason for cancellation

3. Your Full Name
